REG 5- Affadavit for Transfer Without Probate

May be used to transfer ownership of a vehicle when the registered owner or legal owner (an individual) of the vehicle is deceased

REG 101-Statement to record Ownership/ Error

Used to correct error(s) on a vehicle certificate of title during the title transfer process.

REG 135-Bill of sale

Serves as a purchase and sale document in which the buyer and seller record the transaction of a motorized vehicle or vessel.

REG 166- Lien Satisfied/ Title Holder Release

A REG 166 can be used instead of a vehicle Certificate of Title to release ownership. The form is commonly used by lienholders.

REG 227- Application for Replacement or Transfer of Title

A REG 227 is required when requesting duplicate or paperless California Certificate of Title, also known as a Pink Slip.

REG 256- Statement of Facts

A REG 227 is required when requesting duplicate or paperless California Certificate of Title, also known as a Pink Slip.

REG 260- Power of Attorney

This form authorizes a person or entity to sign DMV documents on your behalf and act as your attorney-in-fact.

REG 343- Application for Title or Registration

A REG 343 is required for initial California registration and out of state vehicle title transfers

Breakdown

Line 1a – If only one registered owner is listed under the registered vehicle ownership box, then that person must sign line 1a.

 

Line 1b – If there are 2 parties in the registered owner section of the title, then the 2nd registered owner must sign line 1b. A forward slash(/) indicates that both parties must endorse lines 1a/1b for the title transfer to occur and be legally binding. If an “or” separates the 2 registered owners then either party can release interest in the motor vehicle.
 

Odometer Disclosure – A odometer disclosure is only required if a vehicle is less than 9 model years old. If the vehicle age exceeds 9 model years, the odometer disclosure is not required and can be left blank. The buyer and seller must sign the perspective blocks and fill in the exact mileage of the vehicle. If the mileage is not actual then check the “not actual” box. If the odometer exceeds the mechanical limits then check the “exceeds mechanical limits” box.

 

Line 2 – If the vehicle has a current lienholder the lienholder must endorse line 2 of the certificate of title to release legal interest in the vehicle. If the legal owner is a business the signature must be countersigned. For example, if the business is named Todays Loans the clerk for Todays Loans would sign the certificate of title and then stamp or write Todays Loans above the name.


How To Fill Out An Application for Duplicate of Title


 

SECTION 1- Registered Owner(s) of Record
Start from the line right above Section 1. Enter your vehicle's identification information first. 

Line 1. On the first line of Section 1 write in full name of registered owner
Line 2. On the second line of Section 1 write in the co-owner's full name. If no co-owner exists leave Line 2 blank.
Line 3. On the third line of Section 1 write in your current residence. This must be a physical address. No P.O. box addresses are allowed on this line.

SECTION 2- Legal Owner of Record (Lienholder/Title Holder)
Section 2 applies only if a legal owner (lienholder) exists on the vehicle's registration. Do not enter the name of owner(s) from Section 1. This section is specifically reserved for titles which have a lienholder. If a lienholder is shown on a vehicle's registration card for which full financial obligation(s) has been fulfilled, the DMV will require that the duplicate title application (REG 227) be notarized. 

If no lienholder exists write "NONE" on Line 1 of Section 2.

SECTION 3- Missing Title Statement & Signature

1. Place a checkmark in the check box which best describes the reason for your duplicate title application. If you choose Illegible/Mutilated you must mail in your destroyed title along with your REG 227 application.
2. Print the name of the owner. If more than one owner exists, you may use either name however only that owner will be allowed to sign the REG 227.
3. Sign next to your name. Write in the current date and your daytime phone number.

SECTION 4- Registered Owner(s) Release of Ownership and/or Interest

1. Print the name of the owner. If more than one registered owner exists, you may use the second line to print the name.
2. Sign next to your name. Write in the current date and your daytime phone number.

NOTARY USE ONLY SECTION.
Visit a local notary office and have the REG 227 notarized if you intend on having lienholder information removed from your duplicate title. Notarization is an absolute requirement if a lienholder exists on a vehicle's registration.
